To make a pop-up form in Squarespace, you can follow these steps:
#1. Edit the page you want to add pop-up form > Add a Form Block
#2. Edit the Form Block, at Design, scroll down a bit > Enable the option Lightbox
#3. Result
When clicking on the Open Form pop-up, a form will appear like this
